Health History and Examination Form (Print) |
| Each camp must complete a health history form every year. A current physical (within 24 months of the time of camp) must be on file as well as current immunization records. Parent's, please print this form and fill out the first page and sign it. The second page must be filled out by the camper's physician. |

Medication Form |
| Each camper that has medication to be administered during camp must have a medication form filled out. This form lists all the medications, the dosage, and the times to be administered. Please print this form and fill out and bring on registration day. |

Health History and Examination Form (email) |
| This is a fillable form. You can fill it out online and email it directly to us. The medical exam part of the form must be filled out by a license phyician. Phyicals are given at camp on registration days of Junior Camp and Teen Camp for a $5 fee. You may also get a copy of the camper's most recent phyical from their PCP and mail or fax it to us, or bring it with you on registration day. |
